Game Date: Sat April 27, 2013 vs. Oregon at 6:00pm

With this form, please include a list of names of team members that will be attending with your group. Due to NCAA regulations, High School aged fans will not be admitted unless we have this list.

If you do have high school aged members wishing to attend, please email Ali Miscevich at AMiscevich@athletics.ucla.edu with a roster of names that includes all of the high school aged guests that will be attending with your group. Your request will not be processed without this information.
